Getting Started with CEP Online

Welcome to Hawaii Quality Assurance System Continuing Education Program (CEP) Online!

Please take the time to read through this article to help you get started with our new online program. You should have already signed up for a Google Account, submitted an application to the HQAS Continuing Education Program (CEP), and received approval from the HQAS administrator.

Introduction

The HQAS Continuing Education Program (CEP) Online is designed to be as secure and easy to use as possible. Our site is hosted by Google Sites. The form pages are only accessible to Continuing Education Program applicants using password protected Google Accounts. Never share your Google Account Password with anyone. The Disability and Communication Access Board will never ask you for your password. Our embedded forms are managed through Zoho Creator a secure online form creation and database management website.

Home Page

When you visit our Continuing Education Program home page You will find links to our forms, our Getting Started and Frequently Asked Questions articles, and a calendar of upcoming DCAB workshops.

Program Provider Application

If you are interested in attending a workshop not previously approved by DCAB, please submit the Program Provider Application at least 30 days before the workshop date. Please fill out as much information as possible to expedite the approval process. DCAB will contact the agency or organization's contact person for the rest of the details. After your form has been submitted, you will automatically receive a copy via email. This does not mean your form has been approved, it means our online system has received your submissions. Please keep a copy of this email for your records. Once the workshop has been approved, you will receive another email confirming that the workshop has been approved. You may then attend the workshop. You may be asked to print a Participation Verification Form to be signed by a workshop coordinator. This can be mailed, or scanned and attached to your Tracking Form. Please remember to fill out a Tracking Form submission after you have completed the workshop.

Summary Form

If you are interested in an online workshop, home study, mentorship (being a mentor or mentee), or other forms of independent or participant intiated study, please submit the Summary Form at least 30 days before your intended continuing education activity. After your form has been submitted, you will automatically receive a copy via email. This does not mean your form has been approved, it means our online system has received your submissions. Please keep a copy of this email for your records. Once the educational activity has been approved, you will receive another email confirming your Summary Form has been approved. You may then participate in the continuing education activity. Please remember to fill out a Tracking Form submission after you have completed the continuing education activity.

Mentor/Mentee Questionnaire

Once you have received approval and completed your mentorship, please submit the Mentor/Mentee Questionnaire. Include detailed information including the date, start/end time, goals and evaluation of your mentorship activity. After your form has been submitted, you will automatically receive a copy via email. This does not mean your form has been approved, it means our online system has received your submissions. Please keep a copy of this email for your records.

Participant Verification Form

Please submit a Participant Verification Form each time you complete a CEU activity. Please ensure that all the information you have submitted is correct. If an administrator has requested you print a Participation Verification Form to be signed by a workshop coordinator, you may scan and attach the file to your Tracking Form or submit it by mail. After your online form has been submitted, you will automatically receive a copy via email. This does not mean your form has been approved, it means our online system has received your submissions. Please keep a copy of this email for your records. You will also receive a monthly summary email of your Approved CEUs on the first of the month. To opt out of these monthly emails, please contact the HQAS administrator. If you notice any errors in any of the emails you receive, please contact the HQAS administrator immediately. Please provide as much information as possible as to which form, what was incorrect, and what the corrected entry should be.

Tracking Form

Your Tracking Form is generated automatically based on your Participant Verification Forms. You will receive a monthly Tracking Form Summary by email. The Tracking Form page will allow you to submit a request to have an updated Tracking Form Summary emailed to you right away. Please ensure that all the information you have submitted is correct. If you notice any errors in any of the emails you receive, please contact the HQAS administrator immediately. Please provide as much information as possible as to which form, what was incorrect, and what the corrected entry should be.

Extension Request

If you believe you will be unable to complete your 3.0 CEUs by the deadline, November 30, please submit an Extension Request. Please provide a detailed description of the reason why you are unable to complete the requirement. Please also explain your plan to complete your remaining CEUs by March 31. After your form has been submitted, you will automatically receive a copy via email. This does not mean your form has been approved, it means our online system has received your submissions. Please keep a copy of this email for your records. If you have any problems, please contact the HQAS administrator.

Troubleshooting

I cannot see the forms, what should I do?

If you visit one of the form pages (Program Provider Application, Summary Form, Tracking Form, or Extension Request) and you cannot see the embedded form, but the rest of the site seems to display properly, your web browser may not support or you may have disabled Java Script. Please check your browser settings, advanced settings, or security settings to see if Java Script has been disabled. You may also try using a different web browser application, or a different computer. If your problem persists, please contact the HQAS administrator.

How do I know if my form went through?

After your form has been submitted, a confirmation message with a brief summary of the form you just submitted should appear. Also, an email copy of your form will be sent to you automatically. Please check your email spam folder and any sub-folders where the email could have been accidentally filed. If 24 hours have passed and you still have not received an email confirmation, please submit the form again verifying that your First Name, Last Name, and Email address are spelled exactly as you submitted in your application. If you still do not receive any confirmation, please contact the HQAS administrator.

My Google Account or Email Account has been stolen or hacked!

Notify our administrator immediately. Provide as much information as possible, when you believe your account was stolen, what did you notice that alerted you that your account had been stolen, and what is the best alternate way to contact you. Your stolen account will be blocked. We will attempt to investigate if any of your information has been compromised or altered and notify you. Please be prepared to provide as much information as possible including information from your original application, HQAS certification paperwork, and any information that will help us verify your identity.

Can I submit some of my forms by mail and some forms online?

To insure efficiency and avoid confusion, the Disability and Communication Access Board requires that you choose just one method of submitting forms, either online or by mail. To opt out of the online program and continue submitting forms by mail, please send an email or write a letter to the HQAS administrator requesting to be removed from the online program and submit all your forms by mail.
